Packages expire one year from the date of purchase. Unused treatments will not be refunded after one year.
Discounts for Seniors 60+ & full-time Students with ID (without insurance)

No discounts apply if a patient is using insurance. Only one discount applies for each treatment.

Cupping (cupping is not covered by insurance)
Insurance Plans Accepted
I am an in-network provider for the insurance companies listed below. Please note that not all plans administered by these companies cover acupuncture. Also, coverage will usually require copays or coinsurance payments and be subject to deductibles. If insurance coverage is important to you, please be sure to call your insurance company and ask them to explain your Acupuncture eligibility and benefits before making an appointment with me. Also, please note that I cannot accept Medicare or Medicare Advantage plans. I do offer a Senior 60+ discount, but if you plan to use Medicare or Medicare Advantage please do not schedule an appointment with me. Thank you.
I can accept Ohio Medicaid offered by the plans listed below for a limited number of conditions including low back pain, migraines, cervical (neck) pain, osteoarthritis of the hip, oseoarthritis of the knee, and nausea or vomiting related to pregnancy or chemotherapy. Medicaid patients pay no copays, coinsurance, or deductibles and can usually receive up to 30 acupuncture treatments per year without a referral or prior authorization. Please note that I can only accept Medicaid if it is your primary insurance.
Ohio Bureau of Workers' Compensation
I can accept Ohio BWC patients with completed C-9 forms that have been approved and signed by a managed care organization (MCO). Please contact your physician of record (POR) or BWC case manager for more information about how to request acupuncture services. C-9 forms can be faxed to 614-305-6040, Attention: Mira Choi Tyson, L.Ac.
Health Savings Account (HSA)/Health Reimbursement Account (HRA)/Flexible Spending Account (FSA)
Acupuncture is a qualified medical expense for all HSAs and most HRAs and FSAs. I can accept health account debit cards and checks, or upon request, I can provide a detailed receipt for your records so that you can be reimbursed from your HSA/HRA/FSA for the cost of your treatment.
